No need to "picture this in (your) head". The
video shows it all - except for one step.

Ron appears to be using Open Office with a blank document (or Write) page open.

At second 4 - you see Insert | Picture | From File
At second 5 - you see the dialog box that allows navigation to the desired file
here shown as: "pre-job-application-form-thumb.gif"
At second 7 - the scanned form is now shown as an image anchored to paragraph

** This step is not included. It seems that for
David the Drawing Toolbar is always open. So, next is View | Toolbars | Drawing

At second 9 is the mouse selecting the T - for text box
At seconds 12 to end we see text boxes being created and filled in.

I used the full screen view of the YouTube video to catch all this.
